Enrollment & Eligibility

BENEFIT ELIGIBILITY

All full-time employees working at least 30 hours per week are eligible to participate in all benefits offered by Spooner. Part-time employees working 20-29 hours per week are eligible for all benefits excluding medical. Benefits become effective on the following dates:

 

Salaried Employee:

First day of the month following date of hire

 

Hourly Employee:

First day of the month following 60 days of employment

Eligible Dependents include:

Spouse or domestic partner

Child up to the age of 26 

Children of any age that are totally disabled due to a physical or mental handicap

Once you have made your elections, you will not be able to make any changes until the next annual open enrollment period, unless you experience a Qualifying Life Event (QLE) as defined by the IRS. 

 

QUALIFYING LIFE EVENT (QLE)

If you experience a Qualifying Life Event, you can make a change to your benefit that is consistent with the status change. 

Life Events include:

Marriage

Divorce / Legal separation

Birth / Adoption / Legal custody of a child

 Death of a spouse or dependent child

Employee or dependents gain/loss of other coverage

A change in work status that causes you to gain or lose eligibility.

Dependent Social Security Numbers

Spooner is required by Medicare legislation to collect SSN’s for your enrolled dependents. When enrolling or renewing your coverage, please have this information with you. Please submit SSN for newborns once they are issued.

Forms and documents must be submitted to HR within

30 days of a qualifying event!
